3. ・様式2,様式3及び様式5を除き、各様式はA4判用紙片面1枚となるよう調整すること。
   Each form other than Form 2, Form3 and Form 5 should be adjusted to be one sheet of A4 size paper on one side.
4. ・様式６のオンライン受験を申請する理由を記入する欄は、本学で受験できない理由を記入すること。
   In the space provided for "Reason for applying to take the Online Examination" on Form 6, enter the reason why the examination cannot be taken at the University.
5. ・修士課程修了（修了見込み）証明書，出願資格認定書及び成績証明書は，必ず原本を提出すること。（コピー不可）原本の提出が難しい場合は，下記お問い合わせ先に連絡すること。
   Certification of the Master's Course or of Expected Completion, Qualification Certificate, and Academic Record must be submitted in original. (Photocopies are not acceptable). If you have difficulty in submitting the original, please contact the following address for inquiries.

短期履修制度について

　社会人選抜においては、すでに相当程度研究業績があり、1 年もしくは 2 年以内に博士論文を仕上げることが可能と事前審査で判断された志願者を対象に、標準修学年限以内での学位取得を可能とする「短期履修制度」があります。「短期履修制度」の申請を希望する者は、入学志願票等の提出前の下記日程までに余裕を持って、指導予定教員に申し出て、提出書類及び提出期限を確認してください。
　●10月入学及び4月入学（１次募集）の場合...４月中旬
　●４月入学（2次募集）の場合...7月中旬
